   A Medical Coder is someone who translates details from a patient's medical documents such as physician's notes, lab reports, procedure, an diagnosis into universal medical codes for the purpose of maintaining accurate medical records.  As a medical coder we will use a universal coding system that is recognized throughout Australia. This is the International Classification of diseases , 10th revision Australian Modification, commonly referred to as ICD 10 AM. Clinical coders work mainly at hospitals, but some way more in in specialist research or for health insurance funds.  

Tasks and duties

• Reviewing patients’ medical records to identify all diagnoses, operations, and procedures.
• Clarifying any inconsistencies by consulting with the responsible medical professional and fixing any errors.
• Determining what diagnoses and procedures need to be coded for the patient’s current period of care, then coding them. Diagnoses and procedures need to meet certain criteria for coding, which is in accordance with the relevant standards.
• Maintaining ICD-10-AM coding books to ensure they are updated to the current version being used.

                        

                                               The international version of ICD-10 (International Statistical Classification of Diseases and Related Health Problems) is displayed in the ICD-10 view. There are separate views for the Australian Modification (ICD-10-AM) and the (American) Clinical Modification (ICD-10-CM). The codes are organized based on the hierarchical relationships, they can be expanded down to the four-character subcategories.
